29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) North side of roof, Commons area, above main entrance, on or about 2-21-20: Two employees were working on a sloped 5-1/2:12 pitch roof while connected to retractable cables that were rigged with excess slack, such that the employees could fall more than 6 feet before the retractable could engage and arrest the fall.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Third floor, north side, on or about 2-21-20: Employees were using a portable extension ladder to access a roof that was not 3 feet above the landing surface, in that the top of the ladder was 6 inches above the landing surface, exposing the employees to a fall hazard of 11feet to the floor below.
