29 CFR 1926.405(g)(2)(iii): The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used:  (a) Blue Chip Construction LLC / Blue Chip Housing LLC, on or about February 21st, 2020, at the location of 827 Weadock Ave., Lima, OH, 45804:  The employer did not ensure that employees were protected from shock hazards while using an extension cord that had been damaged and inappropriately repaired so as not to retain its protective properties.  The extension cord was used to power an air compressor and a portable circular saw alternatively.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  (a) Blue Chip Construction LLC / Blue Chip Housing LLC, on or about February 21st, 2020, at the location of 827 Weadock Ave., Lima, OH, 45804:  Employees were exposed to fall hazards of approximately 8 feet while using a ladder inappropriately.  An A-frame ladder was used as an extension ladder, the wrong side was climbed on, and it was not long enough to extend above the upper level being accessed.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   (a) Blue Chip Construction LLC / Blue Chip Housing LLC, on or about February 21st, 2020, at the location of 827 Weadock Ave., Lima, OH, 45804:  Employees working on the ground below roofing activities was not protected from potential falling objects.  Potential falling objects include tools and old roofing materials being removed.   Blue Chip Construction, LLC was previously cited for a violation of this occupational safety and health standard [1926.100(a)(1)] which was contained in OSHA inspection number 1410019, citation number 1, item number 3, and was affirmed as a final order on 8/14/2019 with respect to a workplace located at 833 N. Main St., Lima, OH, 45801.
