29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    (a) Upper Packing Room - When setting up Line 1 and Line 2 loader and packaging machinery at the start of the day and after product changeovers during the day, employees did not follow the established machine-specific lockout/tagout procedures.  The machine-specific lockout/tagout procedure for the loader machinery says to turn off the electrical switch at the back of the machine and apply a lock and lock the main disconnect in the electrical room.  The machine-specific procedure for the packaging machinery identifies electric, compressed air, vacuum, and water as energy sources, and this procedure says to turn off and lockout the energy sources, as follows:  the water at the front of the machine, the electric switch in the electrical room (which also causes the vacuum to bleed out), and the ball valve at the start of the machine.  Employees doing set up on Line 1 and Line 2 only applied lockout/tagout to the emergency stop at each line's operations station and did not lockout/tagout the other energy sources, exposing the employees to hazards such as electrical, struck by, and caught in, on or about February 21, 2020.    (b) Upper Packing Room - When setting up Line 1 and Line 2 loader and packaging machinery at the start of the day and after product changeovers during the day, employees applied lockout/tagout to the emergency stop at each line's operations station, which constitutes control circuitry, rather than applying lockout/tagout to an energy isolating device, such as an electrical disconnect switch, exposing the employees to hazards, such as electrical, struck by, and caught in, associated with unexpected energization or start up, on or about February 21, 2020.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) Upper Packing Room - After setting up machinery at the start of the day and after product changeovers during the day, employees started running products on Lines 1 and 2 and did not install the side guard on each Line's loader head so that the employees could observe the product, exposing employees to the hazard of getting caught in moving machine parts, as occurred on or about February 13, 2020 when an employee grabbed part of the Line 2 loader head, and moving parts of the loader head caught and fractured the employee's finger, resulting in hospitalization.    Corrected during the inspection.    (b) Upper Packing Room - The sets of chains and sprockets, 64 inches above the floor (45 inches above the raised platform on which employees stand), on both sides of each loader conveyor on Lines 1 and 2, located just before each Line's loader head, had no guarding, exposing employees to getting caught in ingoing nip points, on or about February 21, 2020.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

The lockout/tagout program and procedures lacked the specificity needed to enable employees to safely and effectively control hazardous energy when working on particular machines or equipment, exposing the employees to hazards such as electrical, caught in, and struck by, on or about February 21, 2020, as follows:    - The machine-specific procedures for the Upper Packing Room Line 1 and Line 2 loader and packaging machinery did not give the magnitude of the energy sources;    - Neither the general lockout/tagout program nor the machine-specific procedures for the Upper Packing Room Line 1 and Line 2 loader and packaging machinery included provisions for temporarily removing lockout/tagout devices in order to test or position the machinery; and    - Neither the general lockout/tagout program nor the machine-specific procedures for the Upper Packing Room Line 1 and Line 2 loader and packaging machinery specifically identified authorized and affected employees.    Kunzler & Company, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1350905, citation number 1, item number 1b and was affirmed as a final order on December 4, 2018 with respect to a workplace located at 652 Manor Street, Lancaster, Pennsylvania  17603.    Abatement certification and documentation is required within 10 days after the abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
