29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items of this section: Gray (Etch) Room: Employer's energy control procedures for locking out and tagging out the ET-17 Copper Etching Machine did not contain all of the information required by this section. This information included, but was not limited to, the following: a) Specific statement of the intended use of the procedure for ET-17 Machine. b) Specific procedural steps for shutting down and isolating the ET-17 Machine to control hazardous energy such as electrical, pneumatic, and corrosive ferric chloride solution under pressure. c) Specific procedural steps for placement, removal and transfer of lockout tagout (LOTO) devices for ET-17 Machine. d) Specific requirement for testing the ET-17 Machine to determine and verify effectiveness of the LOTO devices.

29 CFR 1910.147(e)(1): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not inspect the work area to ensure that nonessential items had been removed and that machine or equipment components were operationally intact: Gray (Etch) Room: Employer did not ensure that before lockout devices were removed and energy restored to the ET-17 Copper Etch Machine, authorized employees inspected the equipment components of ET-17 machine to ensure the machine was operationally intact. On or about January 14, 2020, an employee was splashed by corrosive ferric chloride solution when a section of piping was not re-installed following repair work before lockout devices were removed and energy restored.
