This information included, but was not limited to, the following:  a) Specific statement of the intended use of the procedure for ET-17 Machine.  b) Specific procedural steps for shutting down and isolating the ET-17 Machine to control hazardous energy such as electrical, pneumatic, and corrosive ferric chloride solution under pressure.  c) Specific procedural steps for placement, removal and transfer of lockout tagout (LOTO) devices for ET-17 Machine.  d) Specific requirement for testing the ET-17 Machine to determine and verify effectiveness of the LOTO devices.

On or about January 14, 2020, an employee was splashed by corrosive ferric chloride solution when a section of piping was not re-installed following repair work before lockout devices were removed and energy restored.
