Employees conducting waste-dumping operations were not properly trained in a safe and standard procedure involving the forklifts, the self-dumping hoppers and how to dump waste.

29 CFR 1910.178(m)(5)(iii): When the operator of an industrial truck was dismounted and within 25 feet of the truck still in his view, the load engaging means were not fully lowered, controls neutralized, and the brakes set to prevent movement. At a worksite located at 1108 Center Road, Rogersville, Missouri 65742, employees were exposed to serious struck-by and crushed-by hazards. Employees engaged in waste-dumping operations were instructed to dismount the industrial truck while the load engaging means were not fully lowered. This was done for the employees to engage a dumping lever located on the back of the self dumping hopper while the hopper was elevated.
