29 CFR 1926.451(g)(4)(ii): The top edge height of top rails or equivalent member on supported scaffolds manufactured or placed in service after January 1, 2000 was not installed between 38 inches (0.97 m) and 45 inches (1.2 m) above the platform surface:   a) Job site, 834 West North Avenue, Pittsburgh, PA: On or about February 24, 2020, during masonry activities just above the top of the 2nd scaffold frame, the employer did not ensure that appropriate fall protection systems/devices were used in that a metal bracket determined to be approximately 30 inches tall from the scaffold planking was used for fall protection, exposing an employee to a fall hazard of up to approximately 12 feet, 8 inches.
