29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  a) Jobsite  The employer did not ensure that an energized (approximately 120 volts) duplex electrical wall outlet was used in accordance with its listing and labeling. The employer allowed employees to energize their tools and equipment with an unmounted and or unsecured wall outlet thus exposing employees to shock and or fire hazards.  Observed on or about 13 FEB 2020.   b) Jobsite - The employer did not ensure that an energized (approximately 120 volts) duplex electrical wall outlet was used in accordance with its listing and labeling. The employer allowed employees to energize their tools and equipment with a wall outlet without a mounted and secured outlet/junction box thus exposing employees to shock and or fire hazards.  Observed on or about 13 FEB 2020.

29 CFR 1926.850(a): Evidence in writing was not available that an engineering survey performed by a competent person had been performed prior to permitting employee(s) to start demolition operations:  a) Jobsite -  The employer did not ensure a written engineering survey was performed on the building before allowing the employees to start interior demolition activities. This condition exposed employees to struck-by and caught-in-between hazards.  Noted on or about 13 FEB 2020.

In each case, any utility company which was involved was not notified in advance.   a) Jobsite - The employer did not ensure that the electrical utilities were shut off and removed to the outside of the building when interior demolition began exposing employees to electrical hazards.  Observed on or about 13 FEB 2020.

29 CFR 1926.850(g): Wall openings in a structure being demolished were not protected to a height of approximately 42 inches:  a) Jobsite - The employer did not ensure that employees throwing debris into dumpsters were protected from falling through wall openings by installing safety bars over the openings, exposing employees to fall hazards of approximately twenty (30) feet.  Observed on or about 13 FEB 2020.

29 CFR 1926.850(k): Employee(s) entrance(s) to multi-story structures being demolished were not protected by sidewalk sheds or canopies or both:  a) Jobsite - The employer did not ensure all entrances and walkways still utilized by employees were protected by canopies from debris that was thrown from second and third floor windows into street-level dumpsters. Employees were exposed to struck-by hazards when walking from the parking locations to the entrance of the second building.  Observed on or about 13 FEB 2020.
