29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.  a) On or about February 14, 2020, located in the Sanitation Department, the employer did not certify that periodic inspections had been conducted for the energy control procedures for machines and equipment, i.e. Line 7 Spiral Infeed Conveyors, thereby exposing employees to a caught-in hazard.

29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  B)  Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;   C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them;  a. On or about February 14, 2020, located in the Sanitation Department, the energy control procedure for the Line 7 Spiral Infeed Conveyor did not include specific procedural steps for safe shutdown, isolation and for the placement of lockout devices. The procedure did not identify what buttons were required to be pushed at the main disconnect or what valve or valves to close and lines to disconnect, thereby exposing employees to caught-in hazards.   The JCG Industries, Inc. Dba Koch Foods was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1910.147 (c)(4)(ii), which was contained in OSHA inspection number 1286896, citation number 02, item number 01 and was affirmed as a final order on February 6, 2018 with respect to a workplace located at 4404 W Berteau Ave. Chicago, IL 60641.

1910.219(f)(3)  Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. 29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  (2) turn off and shut down the machine (3) physically locate and operate the energy isolating devices in such a manner as to isolate the machine or equipment from the energy sources. (4) affix lockout or tagout devices to each energy isolating device (5) Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe. (6) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergizing of the machine or equipment have been accomplished.   a) On or about February 14, 2020, located in the Sanitation Department, employees were exposed to machine hazards associated rotating parts while performing sanitation operations, including cleaning and removal of chicken residue at Line 7 Spiral Infeed Conveyor. The employer failed to implement energy control application steps as the conveyor was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i), and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.  On 2/14/2020 an employee was in the process of cleaning chicken residue from the Spiral Infeed conveyor system for Line 7 while it was running.  As the employee reached under the conveyor system, the employee's rain coat was caught by the rotating sprockets and the employee's hand was pulled in the rotating parts.     Koch Foods was previously cited for a violation of this Occupational Safety and Health standard 1910.147 (d), which was contained in OSHA inspection number 1286896, citation number 01, item number 01 and was affirmed as a final order on February 6, 2018, with respect to a workplace located at 4404 W Berteau Ave. Chicago, IL 60641.
