29 CFR 1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have the load engaging means fully lowered, controls neutralized, and the brakes set to prevent movement: a) On or about 02/07/2020, in the loading dock area, when operator dismounted forklift truck, the slab suspended on the forks of the truck was not fully lowered to prevent employees from accidently being caught under the slab. An employee helping the operator sustained multiple finger amputations when the suspended slab unexpectedly fell on his fingers. ABATEMENT DOCUMENTATION REQUIRED
