29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  a)    Salem VA Medical Center: On or about 02/25/2020, employees required to use chemicals in the workplace such as Henry Blueskin adhesive, were not trained regarding the new label elements and safety data sheets format by December 1, 2013.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
