29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices:  Ammonia Refrigeration Machinery Room East Wall:  On, or about, February 25, 2020, the employer failed to document that the construction of the machinery room complied with the employer's chosen recognized and generally accepted good engineering practices, American Society of Heating, Refrigerating and Air-Conditioning Engineers (ASHRAE) Standard 15-1989, Section 10.14 when the employer failed to tightly seal all pipes and conduit penetrating through the interior walls of the machinery room to the main building.  This hazardous condition exposed workers to toxic hazards in the event of an anhydrous ammonia release.
