All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). The employer was failing to ensure that the HPP machine #3 was properly Locked out/Tagged out using the energy control procedures established for the machine or equipment while performing maintenance or repairs. This was most recently documented on February 25, 2020 at 1601 Pioneers Blvd, Lincoln, NE 68502. The employees were repairing a closure on HPP #3 machine while it was down for maintenance creating a caught-in hazard as the main power source was still on.
