OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck by hazard while using hoists that were not inspected on a regular basis: a) New Salt area, 100 Key Lane, Leechburg, PA 15656: On or about January 28, 2020, periodic inspections of a CM brand hoist (serial number L-18007NN) were not being done. There were no capacity markings on the crane. b) Old Belt area, 100 Key Lane, Leechburg, PA 15656: On or about January 28, 2020, periodic inspections of a 2 ton capacity CM brand hoist (serial number L-661BA) were not being done. The pendant control markings were not legible.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition: a) Shop area, 100 Key Lane, Leechburg, PA 15656: On or about January 23, 2020, Tempering C White W/O containing sodium nitrate (CAS number 7631-99-4) and potassium nitrate (CAS number 7757-79-1) accumulated on the walls, ceiling, and equipment in and near the salt tanks. The accumulation of Tempering C White W/O created a fire hazard and eye irritation hazard to employees working in the area. b) Shop area, 100 Key Lane, Leechburg, PA 15656: On or about January 23, 2020, barrels, crates, and other debris was allowed to accumulate on the floor near a Southerland Press (a panel on the side of the press was labeled 601). The debris and clutter forced forklifts to pass close by the press operator.

Barrier guards on the AIDA (model number NC1-2000(2)(E), serial number CO420-0506) gap frame mechanical press were not properly installed and adjusted. Press operators were exposed to an amputation hazard while using the AIDA press to shock washers.
