The 8 foot Louisville A frame (step ladder) with a Maximum Standing Height of 5 foot 6 inches (as labelled on the ladder) was used to reach a height much greater than the ladders rating.  An employee fell from the ladder to the concrete floor and with the metal Buss Duct Flange/Throat coming down with him.  The employee was hospitalized over the incident.
