(b) Plant 3 - On or about February 26, 2020 the employer did not ensure equipment, such as but not limited to, S1 Slitter were guarded, exposing employees to amputation hazards. (c) Plant 4 - On or about February 26, 2020 the employer did not ensure equipment, such as but not limited to, VPM 1 CNC Foam Cutter, VPM Offload Conveyor were guarded, exposing employees to amputation hazards. (d) Plant 5 - On or about February 26, 2020 the employer did not ensure equipment, such as but not limited to, No. 1 and No. 2 Slitters, B1, B2, B3 Vertical Bandsaws, Carrousel Cutter were guarded, exposing employees to amputation hazards.
