On February 26, 2020, and times prior thereto, the employer did not ensure a competent person completed a visual inspection with the rough-terrain crane prior to each shift the crane was in service.  This exposed the employees to struck-by hazards.

On or about September 18, 2019, the employer did not document and maintain an initial equipment inspection which contained: a) the date of the inspection; b) the items checked; c) the results of the inspection, and; d) the name and signature of the person who conducted the inspection, for the rough-terrain crane.  This exposed employees to struck-by hazards.
