29 CFR 1910.157(g)(2): The employer did not provide the education required in paragraph (g)(1) of this section upon initial employment and at least annually thereafter:      (a) Scottdel Cushion, LLC, on or about February 21st, 2020, at the location of 413 Zeiter Dr., Swanton, OH: Employees were not provided with both initial training and annual training for portable fire extinguisher usage.  Employees were exposed to potential burn hazards and smoke inhalation hazards from emergency fire fighting.

Employees were exposed to burn and smoke inhalation hazards through the use of welding equipment such as mig-welders.
