29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:   On or about February 26, 2020, at a worksite located at 160 Vicary Square Northeast, Canton, OH 44714, during residential roofing activities on a steep sloped roof, the employer did not ensure employees were protected from eye hazards by ensuring eye protection was available and worn while operating pneumatic nail guns at approximately 120 psi.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about February 26, 2020, at a worksite located at 160 Vicary Square Northeast, Canton, OH 44714, during residential roofing activities on a steep sloped roof, employees were exposed to a fall of approximately 20 feet. The employees were using an extension ladder to access the roof that did not extend at least three feet above the landing area.
