29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59)  At the residential construction site located at 2011 NE 113th Terrace in Kansas City, Missouri:  Employees engaged in roofing activities were exposed to health hazards from dermal, ingestion, and inhalation exposure to hazardous chemicals.  The employer had not developed or implemented a written hazard communication program to ensure that employees were aware of the hazards associated with the chemicals with which they work.  Hazardous chemicals used at the site included but were not limited to residential sealant.

Employees were observed installing materials and working at height.  Employees were exposed to a fall distance of greater than 20 feet eave to ground without use of fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the residential construction site located at 2011 NE 113th Terrace, in Kansas City, Missouri: Employees were exposed to fall hazards. Employees were observed accessing and egressing the roof by way of a portable ladder that did not extend 3 feet past landing.  Employees were exposed to a fall distance of greater than 10 feet eave to ground.
