OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that the employees were exposed to struck-by hazards from falling stock. On or about February 25, 2020, the employer exposed employees to struck-by hazards by falling product/racks as sections of the racking system of all column base plates of the storage racks were not anchored to the floor with anchor bolts, approximately half of the racks in the facility were not anchored in the warehouse.

Back strain (lumbar) while lifting items at work and was prescribed medication. Date of incident 12/31/19. b. Back strain (disc) while lifting items at work and prescribed prescription strength medication. Date of incident 2/13/19.
