29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) Job site, 1240 Sawmill Run Boulevard, Pittsburgh, PA: On or about February 26, 2020 during block laying activities, the employer did not ensure that appropriate fall protection systems/devices were in use along the end of a scaffold end frame exposing employees to fall hazards of up to approximately 32 feet.

29 CFR 1926.451(g)(4)(xv): Cross bracing at the crossing point of two braces was between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform and no top rail was in use: a) Job site, 1240 Sawmill Run Boulevard, Pittsburgh, PA: On or about February 26, 2020 during block laying activities, the employer did not ensure that appropriate fall protection systems/devices were in use in that employees used only cross bracing as fall protection where the cross brace was low enough to serve as only a mid rail and a top rail was not provided, exposing employees to a fall hazard of up to at least approximately 32 feet.
