29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about February 27, 2020, at the jobsite, the employer did not ensure that employees were in use of protective helmets when working from lower levels in areas where employees worked above.  This exposed the employees to struck-by hazards.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:   On or about February 27, 2020, at the jobsite, the employer did not ensure that employees were in use of eye protection while using hammers and pneumatic nail guns to install roofing materials.   This exposed employees to struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about February 27, 2020, at the jobsite, the employer did not ensure that employees were in use of fall protection when working on the roof of residential building that was six feet or more to a lower level.  This exposed the employees to a fall hazard greater than six (6) feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   On or about February 27, 2020, at the jobsite, the employer did not ensure that the side rails of a portable ladder extended at least 3 feet (.9 m) above the upper landing surface and tied off at the top to prevent displacement.  The employees used the ladder to access and exit the roof and were exposed to a fall hazard.
