29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about February 25, 2020, and at times prior thereto, employees on the roof of a newly constructed commercial building located on Wilson Rd, in Conroe, TX, were exposed to a struck-by hazard while operating pneumatic nail guns and handsaws without wearing eye protection.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide:   On or about February 25, 2020, and at times prior thereto, at a newly constructed commercial building on Wilson Rd, in Conroe, TX, employees located:   a.  On the East side of the building, were exposed to a fall hazard while working on a scaffold platform / walkway that was not 18 inches wide.  b. On the South side of the building, were exposed to a fall hazard while working on a scaffold platform / walkway that was not 18 inches wide.

Crossbraces were used as a means of access.  On or about February 25, 2020, and at times prior thereto, on the East side of a newly built commercial building,  employees were exposed to a fall hazard when performing framing activities from a fabricated frame scaffolding without a ladder or other safe means of access.

On or about February 25, 2020, and at times prior thereto, on the East side of a newly constructed commercial building, employees were working on a scaffold, installing framing, without having been trained to recognize and minimize hazards associated with the scaffold.
