29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly:   U.S. Forest Service Office, Mr. Baker Snoqualmie Supervisors Office located at 2930 Wetmore Avenue Everett, WA:  (a)  Southwest Exit;  The First Alert fire extinguisher located adjacent to the exit door was not visually inspected monthly.   (b)  Northwest Exit;  The Ansul Sentry fire extinguisher located adjacent to the exit door was not visually inspected monthly.   (c) Dispatch Office;  The ABC fire extinguisher was not visually inspected monthly.   This exposes employees to possible smoke inhalation hazards if they try to extinguish a small fire and the fire extinguisher is inoperable.   Note:  Applies to any extinguishers provided in the workplace.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1067761, citation number 1, item number 4 and was affirmed as a final order on October 15, 2015, with respect to a workplace located at Trout Lake, WA.

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.   U.S. Forest Service Office, Mr. Baker Snoqualmie Supervisor's Office located at 2930 Wetmore Avenue Everett, WA:   (a)  Supervisors Office located on West side of building;  A Sunbeam heater was plugged into a surge protector (relocatable power tap).  CORRECTED DURING INSPECTION   (b)  AQM Cubicle on 4th Floor;  A surge protector power strip was plugged into another surge protector power strip which was powering Dell and HP Computer equipment.   (c)  Dispatch Office on 1st Floor;  A GE microwave was plugged into a surge protector power strip (relocatable power tap). CORRECTED DURING INSPECTION   The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.     The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.    This exposes employees to possible electrical fire hazards.       The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1438911, citation number 2, item number 1 and was affirmed as a final order on February 3, 2020, with respect to a workplace located at North Bend, WA.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1315293, citation number 1, item number 4 and was affirmed as a final order on September 26, 2018, with respect to a workplace located at Wenatchee, WA.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1214195, citation number 1, item number 1 and was affirmed as a final order on May 15, 2017, with respect to a workplace located at Olympia, WA.
