29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:               a) On or about February 28, 2020, employees were exposed to the hazard of falling from elevations up to approximately 12 feet when climbing portable ladders while carrying rolls of roofing felt and bundles of shingles to a roof deck landing surface.        In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:        a) On or about February 28, 2020, a roof anchor used as part of a fall protection system was not connected completely to the roof deck to ensure the device would be capable of supporting at least 5,000 pounds of force.         Keith Tilden Roofing Company dba Tilden Roofing Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.502(d)(15) which was contained in OSHA inspection number 1142903, citation 2, item 1, and was affirmed as a final order on August 2, 2016, with respect to a worksite located at 924-930 Ashland Ave., River Forest, Illinois.          In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
