29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.) 94 Prospect Point Rd. Lake Hopatcong, NJ   Employees engaged in construction activities were exposed to an approximate 20ft. fall hazard when utilizing an extension ladder that was only extended approximately 1 inch above the landing surface accessed, on or about 02/28/2020.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:  a.) 94 Prospect Point Rd., Lake Hopatcong, NJ   Employees were exposed to fall hazards while utilizing an A-Frame ladder in the closed position in order to access the next highest working level, on or about 02/28/2020.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a.) 94 Prospect Point Rd. Lake Hopatcong, NJ   Employees working underneath construction activities were not provided with hard hats to protect them against head injuries from falling objects overhead, on or about 02/28/2020.  Blue Eagle Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1272186, citation number 1, item number 1 and was affirmed as a final order on 03/12/2018, with respect to a workplace located at 35 North Rd., Chester, NJ 07930 .

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)  NOTE TO CSHO: USE THIS OPTION ONLY IN CASES WHERE THE INFEASIBILITY/GREATER HAZARD EXCEPTION IS NOT IN ISSUE.   a.) 94 Prospect Point Rd. Lake Hopatcong, NJ   Employees without fall protection were exposed to an approximate 20 foot fall hazard while engaged in construction activities, on or about 02/28/2020.  Blue Eagle Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1272186, citation number 1, item number 2 and was affirmed as a final order on 03/12/2018, with respect to a workplace located at 35 North Rd., Chester, NJ 07930.
