On or about 02/27/2020. a) Employees were dismantling the scaffolding was not erected, installed and maintained per the Engineer's design. The scaffold was constructed and maintained by the employer and had not installed and maintained the tiebacks, did not install and the outrigger beams and the specific plans for the beam stability per the design.

29 CFR 1926.451(c)(1)(ii): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by installing guys, ties, and braces according to the manufacturer. Site: 535 Grand Street in Brooklyn, NY. On or about 02/27/2020. a) Employees were dismantling the scaffolding that had tiebacks removed from the top and lower sections. The top section tiebacks were previously removed and the stucco patches were repaired and painted, the lower sections were also removed and the tieback were partially patched.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. On or about Feb. 27, 2020 At 535 Grand St. Brooklyn NY A competent person did not supervise the dismantling of a tubular welded frame scaffold erected on the side of the construction site. The scaffold was not installed and maintained with appropriate tiebacks and supports per the plans designed by the Engineer. Employees were exposed to crushing injuries when the scaffold collapsed during the dismantling of the scaffold.
