29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:    (a) Apex Systems at 151 Detroit Street in Denver, CO 80206: On and before February 19, 2019, the employer did not report a two finger amputation of an employee which occurred on his jobsite at Janus Henderson Investors on February 6, 2019 at approximately 9:30am.  The injured employee was unloading equipment from loading dock when his left pinky and middle fingers were caught between the dock plate and the dock platform.  This condition exposed employee to amputation injury which was not reported to OSHA within 24-hours as required.
