29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition.    a) Chip Thickness Screening Building (Chip Reclaim Area) - On February 29, 2020, and at times prior to, regular inspections of the walking working surfaces including the equipment accesses and fall protection systems of the accesses were not completed to ensure the areas were maintained in a safe condition exposing employees to falls.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.29(b)(5): The employer did not ensure that midrails, screens, mesh, intermediate vertical members, solid panels, and other equivalent intermediate members are capable of withstanding, without failure, a force of at least 150 pounds (667 N) applied in any downward or outward direction at any point along the intermediate member.     Chip Thickness Screening Building (Chip Reclaim Area) -  On February 29, 2020, the employer did not ensure the midrail of the guardrail system in the chip reclaim area of the fourth floor was capable of withstanding, without failure, a force of at least 150 pounds applied in any outward direction exposing employees to falls.        Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
