29 CFR 1926.1053(b)(7): Ladders were used on slippery surfaces:    (a)  On or about March 2, 2020, a Keller 24' extension ladder being used for the purpose of siding installation, was set-up and utilized on frozen and ice covered ground. This exposed an employee to a fall hazard of 8-10 feet.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.  (a) On or about March 2, 2020, a Keller 24' extension ladder being used for the purpose of siding installation was set-up and utilized on frozen and ice covered ground. The extension ladder was not secured at the top or the bottom of the ladder to prevent accidental displacement when in use. This exposed an employee to a fall hazard of 8-10 feet.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.    (a)  On or about March 2, 2020, an employee working from a Keller 24' extension ladder had not received training necessary to recognize hazards associated with ladder use and procedures to be used to minimize the hazards of ladder use.
