29 CFR 1910.28(b)(9)(i)(A): Each fixed ladder that extended more than 24 feet above the lower level and was installed before November 19, 2018 was not equipped with a personal fall arrest system, ladder safety system, cage, or well: On or about February 29, 2020, the fixed ladder installed inside the GSI model 5000 tower dryer extending from the lowest interior work platform more than 24 feet to the second level work platform inside the dryer, and the ladder was not equipped with a personal fall arrest system, ladder safety system, cage, or well.
