29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On the east side of the construction site, employees engaged in decking activities used pneumatic nail guns and were not provided eye and face protection. The employees were exposed to struck-by hazards and eye injuries.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide. On the east side of the construction site, employees engaged in cornice activities on a residential home from a single pole scaffold worked from a platform that was not 18 inches wide. The employees were exposed to fall hazards of up to 6 feet to the lower level.
