29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed: Warehouse: Employees perform material handling operations, such as breaking down loads and separating products onto pallets, without an assessment performed to identify the hazards that necessitate personal protective equipment. Employees were exposed to lacerations and wood splinters when working with wooden pallets without proper gloves readily available.

Warehouse: Employees perform material handling operations, such as breaking down loads and separating products onto pallets. An employee was identified wearing gloves that were in disrepair. On both gloves, the fingertips were torn and the rubber coating was worn away exposing the fingertips. Employees were exposed to lacerations and wood splinters.
