29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): 491 Pleasant Street, Malden, MA 02148 a) On or about 3/3/20, employees working on the roof of the house removing the old roof were not protected by a personal fall arrest system. Employees were wearing harnesses that did not extend above the waist, and were not attached to ropes placed on the roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: 491 Pleasant Street, Malden, MA 02148 a) On or about 3/3/20, employees working on the roof of the house removing the old roof did not extend a ladder at least 3 feet above the roofline. The ladder was extended so that the top rung was at or just above the roof surface.
