29 CFR 1915.1026(d)(1): The employer with a workplace or work operation covered by this standard did not determine the 8-hour time-weighted average exposure for each employee exposed to chromium (VI):  a)    Building 59: On or about 02/28/2020, the employer did not determine the 8-hour time-weighted average exposure to hexavalent chromium for an employee required to weld together stainless steel ladders, as a result the employee was possibly exposed above the action or permissible exposure level.   Note: The employer is to make this determination in accordance with either paragraph (d)(2), the scheduled monitoring option, or paragraph (d)(3), the performance-oriented option.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1960.8(a):The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm in that an employee's right thumb was exposed to a caught-between hazard that resulted from a steel plate wedged between the work rest and grinding surface of a Yates American Machine 16-inch Model #J-147 disc sander. According to American National Standard B11.9-2010, section 8.3.2.2, a 1/8-inch clearance was required between the abrasive product and the work rest:  a)    Building 59: On or about 02/27/2020, an employee used a Yates American Machine 16-inch disc sander Model #J-147 to deburr metal plates. The distance between the work rest and abrasive product exceed 1/8-inch gap, as a result a portion of the employee's right thumb was amputated.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.212(b): Machine(s) designed for fixed location(s) were not securely anchored to prevent walking or moving:  a)     Building 59: On or about 02/28/2020, the Yates American Machine Model #J-147 16-inch disc sander was not anchored to the floor to prevent movement use.  No abatement certification or documentation required.
