29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):    On or about February 14, 2020 at the TECT Aerospace plant located at 5545 N. Mill Heights Drive Park City Kansas, the employer failed to adequately develop written lockout/tagout procedures for employees to follow while troubleshooting the W/C 188C Niigata SPN 50 CNC Horizontal Mill machine, thus exposed employees to struck-by and crushing hazards. The equipment specific lockout/tagout procedures did not include:  (A) A specific statement of the intended use of the procedure, and  (B) The specific procedures for the temporary removal or transfer of lockout/tagout devices for the purpose of troubleshooting, testing or positioning of machine/equipment.    29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is complete.

29 CFR 1910.147(c)(6)(ii): The employer did not certify that the periodic inspections have been performed:    Employees engaged in servicing, maintenance, setup, clearing jam, and troubleshoot activities throughout the machine shop area at the following equipment but not limited to the W/C 186D Niigata SPN 50 H.O CNC machine, the W/C 188C Niigata SPN 50 CNC machine, the Makino Mag 3 EX machine, the Makino Mag 3 W machine, and the Powermill H18 Series machine which had multiple energy sources including pneumatic, electric, hydraulic, and gravity were exposed to electrical, struck-by and caught-between hazards in that periodic inspections of the energy control procedures for equipment were not performed.    29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.1200(j)(2)(ii): The employer did not update any alternative workplace labeling used under paragraph (f)(6) of this section, update the written hazard communication program required by paragraph (h)(1) to reflect changes in labeling and safety data sheets, and/or provide any additional employee training in accordance with paragraph (h)(3) for newly identified physical or health hazards no later than June 1, 2016:    Employees engaged in servicing, maintenance, cleaning, machining, deburring, assembly activities throughout the machine shop area were exposed to chemical hazards in that the plant current written hazard communication program had not been updated in accordance with the revised hazard communication standard. Employees using hazardous chemicals, including but not limited to: steel fume, coolant, gasoline, propane, and solvent.    29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  Employees voluntarily using 3M N95 respirators while cleaning, machining, assembling, and deburring were not provided the information contained in Appendix D.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
