29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.  On or about March 5, 2020, at the above addressed jobsite, an employee working on the ground, below roofing activities, was not wearing a hard hat to protect against falling materials and debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a. On or about March 5, 2020, at the above addressed jobsite, employees performing residential roofing operations on a roof nineteen (19) feet three (3) inches and nine (9) feet above ground were not protected from falls, thereby exposing employees to fall hazards.  Quality Construction Pros Inc.  was previously cited for a violation of this Occupational Safety and Health standard 1926.501(b)(13), which was contained in OSHA inspection number 1322063, citation number 2, item number 1 and was affirmed as a final order on September 6, 2018, with respect to a workplace located at 1651-59 W. Cortez St., Chicago, IL 60622.  Quality Construction Pros Inc.  was previously cited for a violation of this Occupational Safety and Health standard 1926.501(b)(13), which was contained in OSHA inspection number 1303591, citation number 1, item number 3 and was affirmed as a final order on May 31, 2018, with respect to a workplace located at 5820 S. Nordica Ave., Chicago, IL 60638.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On or about March 5, 2020, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.  Quality Construction Pros Inc. was previously cited for a violation of this Occupational Safety and Health standard 1926.1053(b)(1), which was contained in OSHA inspection number 1322063, citation number 1, item number 1 and was affirmed as a final order on September 6, 2018, with respect to a workplace located at 1651-59 W. Cortez St., Chicago, IL 60622.
