29 CFR 1926.453(b)(2)(v): A safety harness with lanyard attached to the boom or basket was not worn by employees working from an aerial lift: At work site located at 13441 Holmes Rd, KC MO -- Two employees were in a SkyJack 45T aerial lift and did not have a safety harness with lanyard attached to the boom or basket. This exposed employees to fall hazards of approximately 20 feet to the ground and to be catapulted from the aerial lift. This was documented on or about 03/05/2020.
