(2)  Challenger Lift, model Q10000, serial #080800000011200, did not have the locking mechanism locked             into place while the lift was raised and in use.                   (3)  Auto lifts were not periodically inspected by a qualified inspector at least annually to ensure that             they were properly maintained and functioning.            (4)  The employer did not document that all lift users were trained to use the lifts.
