OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing and struck-by hazards while using an automotive lift:    (a)  On or about March 4, 2020, and at times prior the employer did not ensure that employees         were safe from crushing and struck-by hazards in that the following lifts were noted with deficiencies:        (1)  Rotary Lift, model 010000, serial #080800000011201, did not have the locking mechanism locked             into place while the lift was raised and in use.        (2)  Challenger Lift, model Q10000, serial #080800000011200, did not have the locking mechanism locked             into place while the lift was raised and in use.                   (3)  Auto lifts were not periodically inspected by a qualified inspector at least annually to ensure that             they were properly maintained and functioning.            (4)  The employer did not document that all lift users were trained to use the lifts. 29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.  (a) On or about March 4, 2020, and at times prior, the employer, did not ensure employees in the service area were protected from chemical hazards. Numerous secondary containers including, but not limited to Clear Choice GoldPlus Antifreeze, Non-Hexane Brake Cleaner, and AC-Delco Dex-Cool Antifreeze/Coolant, did not have product identifiers on them to alert of hazards associated with each chemical.
