29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Production department (Line 9) - On or about 2/26/2020, and at times prior to, the employer exposed employees to amputation hazards in that no method of machine guarding was provided to protect the operators when employees were operating detabber machine (Johnson Controls, Inc. - Model # Liberty St. -100 Trimmer & Serial # 4931T).

29 CFR 1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard that the employees had not previously been trained about was introduced into their work area. Information and training was not designed to cover all applicable categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals: a) Production Department (Line 9) - On or about 3/5/2020, and at times prior to, the employer exposed employees to skin, eye and respiratory system irritations in that employees were not provided with effective information and training on hazardous chemicals at their initial assignment when they were working with hazardous chemicals such as, but not limited to, resin (High Density Polyethylene).
