29 CFR 1926.304(d): 29 CFR 1926.304(d): Each portable, power-driven circular saw withdrawn from the work did not automatically and instantly return the lower guard to the covering position of the saw. a) On or about March 6, 2020, on the roof of a new construction home, employees were using a Dewalt circular saw (serial # unknown) to cut lumber to install the roof frame of a home with the lower guard blocked with wedges preventing the proper function of lower safety guard.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. a) On or about March 6, 2020, on the roof of a new construction home, employees were using an extension cord that had had not been inspected for strain relief damage to the outer shield at the plug and damage to sections of the outer sheathing.
