29 CFR 1926.25(a):   During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas:  a)  155 Ridgedale Avenue, Florham Park, NJ.  Employees were exposed to trips and falls and puncture wounds as many pieces of wood with protruding nails were strewn about the first floor of the new home under construction.                                                  Violation observed on or about 03/6/2020.

29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:   a)  155 Ridgedale Avenue, Florham Park, NJ.  Employees were exposed to penetrating eye trauma as they were using or working in close proximity to the pneumatic coil nailer and not using eye protection. 29 CFR 1926.405(g)(2)(iv):   Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:   a)  155 Ridgedale Avenue, Florham Park, NJ.  Employees were exposed to electric shock as the female end of one extension cord and the male end of another extension cord had strain showing where they plugged into the 3 outlet power tap.                                                          Violation observed on or about 03/07/2020.

Employees were exposed to electric shock as the temporary cord or extension cord which powered the portable circular hand saw was frayed in at least 3 areas. 29 CFR 1926.501(b)(13):   Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 155 Ridgedale Avenue, Florham Park, NJ.  Employees involved in framing the new home were exposed to falls of 12 1/2 feet to the ground below from the second floor's eave as fall protection was not being used.  In addition, workers were exposed to falls through a floor hole of 10'5" to the basement below as no guardrails were across an opening and to falls of 8'9" out a window opening as it was not guarded properly.                                                     Violation observed on or about 03/06/2020.
