29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems or personal fall arrest systems:  Instance a. Site - On or about March 5, 2020, employees engaged in framing activities from the garage roof area were not provided with fall protection and were exposed to falls of over 10 feet.  Instance b. Interior second floor level - On or about March 5, 2020, employees were exposed to fall hazards where window openings were not guarded or blocked to prevent falls of over 10 feet.   Instance c. Interior second floor level - On or about March 5, 2020, employees were exposed to fall hazards where doors openings were not guarded or blocked to prevent falls of over 10 feet.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with one stairrail system along each unprotected side or edge:  Interior staircase - On or about March 5, 2020, the staircase utilized by the employees to access the second story work area was not provided with stair railing along the open sided staircase to prevent employees' fall hazards.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not marked in a manner that readily identified them as defective, tagged with "Do Not Use" or similar language, or withdrawn from service until repaired:  Garage area - On or about March 5, 2020, the 8 foot (Werner) stepladder utilized for roof access was not adequate, where the ladder was broken and missing the rear support system for the ladder exposing the employees to a fall hazard.
