On or about 3/6/2020, at the Safety 1st Playground Inc. jobsite located in the yard area of Fox Trail Elementary,1250 S Nob Hill Rd, Davie FL 33324. Two worn slings were used for handling a metal canopy structure, exposing employees to a potential struck by hazard.

An employee was not trained in proper procedures to setup ladders, exposing employee to an approximate 12-feet fall hazard while working from a ladder which was not properly secured.
