29 CFR 1926.453(a)(1): Aerial lift was not designed or constructed in accordance with the applicable requirements of the American National Standard for "Vehicle Mounted Elevating and Rotating Work Platforms," A92.2-1969: (a) On or about March 03, 2020, in the front of the building, the employees were using an aerial lift (JLG, model G12-55A) to raise employees up to the roof without a properly designed manlift.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) On or about March 03, 2020, in the front, west end of the building, the employees were using a rough terrain lift truck (JLG G12-55A) to raise them up approximately 12ft to work on the roof.
