29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation were performed by the customer or user without manufacturer's prior written approval: Plant - On or about 03/04/20 and at times prior thereto, powered industrial trucks including (but not limited to) the Doosan 25 LPG-powered forklift were operated with the front racks removed. Or in the alternative 29 CFR 1910.178(q)(6): Industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer, or were altered either by the addition of extra parts not provided by the manufacturer or by the elimination of parts, except as provided in paragraph (q)(12) of this section. Plant - On or about 03/04/20 and at times prior thereto, powered industrial trucks including (but not limited to) the Doosan 25 LPG-powered forklift were operated with the front racks removed. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Plant - On or about 03/04/20 and at times prior thereto the rollers in the sheet extrusion/lamination line were not protected to prevent accidental contact with the ingoing nip points. Or in the alternative: 29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Plant - On or about 03/04/20 and at times prior thereto the rollers in the sheet extrusion/lamination line were not protected to prevent accidental contact with the ingoing nip points. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
