29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about March 9, 2020, at a worksite located at 619 Grant Avenue, Cuyahoga Falls, OH 44221 during residential roofing activities employees were exposed to fall hazards of approximately 10.8 and 19.5 feet. The employer failed to ensure employees who were exposed to fall hazards were trained in recognizing fall hazards or the procedures to minimize the hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about March 9, 2020, at a worksite located at 619 Grant Ave, Cuyahoga Falls, OH 44221 during residential roofing activities, the employer did not ensure the portable ladder extended at least 3 feet above the upper landing surface.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   On or about March 9, 2020, at a worksite located at 619 Grant Avenue, Cuyahoga Falls, OH 44221 during residential roofing activities the employer did not ensure all employees exposed to head impact injuries from falling or flying debris were provided head protection.  JJs Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1386186, citation number 01, item number 001 and was affirmed as a final order on April 22, 2019, with respect to a workplace located at 321 Whipple Ave, Canton, OH 44704.
