29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Employees were exposed to fall hazards.  Employees engaged in residential roofing activities were exposed to fall hazards of approximately 12 feet from the roof to the ground in that fall protection was not provided.    The MC Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1336910, citation number 1, item number 1 and was affirmed as a final order on 09/07/2018, with respect to a workplace located at 10938 Hidden Prairie Parkway, Fountain, CO 80817.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   Employees engaged in residential roofing activities were exposed to falls from approximately 12 feet, in that the aluminum ladder used to access the roof did not extend 3 feet from the eave of the roof.   The MC Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1336910, citation number 1, item number 2 and was affirmed as a final order on 09/07/2018, with respect to a workplace located at 10938 Hidden Prairie Parkway, Fountain, CO 80817.
