29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:   On or about March 2, 2020, at the jobsite, the employer did not ensure that employees were in use of a protective helmet while working from lower levels below the areas where other work was being performed.  This exposed the employees to struck-by hazards.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:   On or about March 2, 2020, at the jobsite, the employer did not ensure that employees were in use of eye protection while using hammers and pneumatic nail guns to install roofing materials on the roof of a building.   Employees were exposed struck-by hazards.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   On or about March 2, 2020, at the jobsite, the employer did not ensure that an employee, operating a powered industrial truck (PIT), was trained and evaluated prior to use of the PIT.  This exposed employees to struck-by hazards.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   On or about March 2, 2020, at the jobsite, the employer did not ensure that employees, performing roofing work activities from the steep roof of a building, were in use of fall protection.  This exposed the employees to a fall greater than six(6) feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   On or about March 2, 2020, at the jobsite, the employer did not ensure that the side rails of a portable ladder extended at least three (3) feet above the upper landing surface and was not secured to prevent displacement.  This exposed employees to a fall hazard.
