Site: Road at the corner of 57th Street and 8th Ave in Brooklyn, NY. On or about 03/09/2020. a) The competent person did not inspect the trench after it was dug 5 feet deep and before employee entered the trench.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). Site: Road at the corner of 57th Street and 8th Ave in Brooklyn, NY. On or about 3/09/2020. a) Employees were working in a trench more then 5 feet deep that did not have any protective system installed. Work was done on the pipes in the trench.
